Community Health Needs Assessment

We identify the health needs of our community in a number of ways. First and foremost, we listen to our patients.  We also consider data, such as that collected in our Community Health Needs Assessment (CHNA), which is conducted every three years.  We welcome input and seek feedback from various groups, such as the Community Advisory Council, made up of over 50 local residents representing all communities in our service area.  To the fullest extent possible, we respond to specific requests from our community partners.
