Lost Vaccination Card

What to do if you’ve misplaced or lost your CDC Vaccination Card
 
First, please know that Northern Light Health is not able to reproduce CDC Vaccination Cards. We will be happy to help you receive a paper record of immunization, which is perfectly acceptable as a replacement for the card.  Please see below for additional information and steps you can take.
 
  1. I received my vaccine from a Northern Light Health facility and have lost my CDC Vaccination Card.  What do I do?
    1. If you are a patient of Northern Light Health and enrolled in our patient portal, you may go to the mynorthernlighthealth.org patient portal and download the information from there. 
    2. If you are new to the patient portal, you may enroll by clicking here. Please be advised that to complete enrollment you will need to know your medical records number, which can be obtained via calling the Support Line at 1-833-217-9640.
 
  1. I am a Northern Light Health patient, and received my vaccination elsewhere. I have lost my CDC Vaccination Card.  What do I do?
    1. You may call your Primary Care Provider.
    2. If you are enrolled in the mynorthernlighthealth.org patient portal you may also download the information from there. If you are not registered for our patient portal, you may register for it here.
 
  1. I received my vaccine from a Northern Light Health facility but I am not a patient of a Northern Light Health provider.  I  have lost my CDC vaccination Card.  What do I do?
    1. You may also enroll in our mynorthernlighthealth.org patient portal by clicking here. You do not need to have a provider at Northern Light Health to enroll and use our patient portal.
