Telemonitoring

Telemonitoring uses information technology to remotely monitor your health status. A device would collect clinical information such as blood pressure or respiration while you are home.

An example of telemonitoring includes remote evaluation and diagnosis of sleep disorders. Other examples include wearable fitness devices that record your heart rate and activity such as steps walked.

People living at home or in a facility are monitored for a wide range of chronic or post-discharge conditions by a clinician by two-way video. Our staff make assessments and recommendations to stabilize your condition and reduce the chances of an emergency room visit.
